Your name (first and last):
Email address (fully qualified):
Office phone:

Notes:
1. The reservation keycode specified must match the value earlier presented to the bookpwd program. If you forget your password or have not yet established one, execute the command

The bookpwd program is not available on any of the DB research servers or workstations.
2. As a convenience your name, phone, email, and reason for booking rooms may be permanently recorded in the file ~/.bookingrc within your home account on the cs-general environment. Simply create lines in this file having the above identifiers, a colon, and then the desired default value. Make sure this file is globally readable.
3. The user who creates a booking can modify it, as can any individual who is a member of group www_db.
4. The userid and reservation keycode field are the only field required when modifying or deleting bookings.
5. You must specify the total hours a booking covers when it is being modified.
6. When deleting a booking, you must specify at least one time slot covered by the booking.
